Service Description

Upon booking, you'll receive my intake form that I highly recommend you complete to the best of your abilities and send back at least 24 hours prior to our appointment in order to get the most out of our time together. Following our discussion I'll send you an email with detailed notes, recommendations, diet/exercise plans (if applicable), and any other important information that we discussed, and I'm also available for any follow up questions as required. Please note, price is in USD.
